CarePlus Health Mixup Discloses Patient Data of 11,200

Miami-based CarePlus Health Plans has disclosed that Explanation of Benefits (EoB) statements mailed on two dates in January 2018 accidentally provided protected health information (PHI) of certain plan members to other members.

In total, approximately 11,200 plan members are believed to be impacted. CarePlus said that a series of programming and printing errors led the EoB statements to be sent to the wrong individuals.

Breach notification letters have been sent to impacted plan members. It is the second mismailing incident by CarePlus Health Plans in the past three years. CarePlus previously announced that it mailed an extra EoB with health information for more than 1,400 different members to other members in September 2015.
