White and Bright Family Dental disclosed to patients and the public about unauthorized access to a business computer server with protected health information. The server contained personal information including patient name, address, telephone number, social security number, data of birth, driver license number, insurance information and dental history.
The letter from Salih M. Mayalidag, D.D.S., did not indicate the number of patients that were impacted. The dentist indicated only that they believe the information was accessed and do not have knowledge about whether it was copied or stolen.
The Fresno Police Department has been notified; the police report number is 18300943. The letter recommended review of health and financial statements for accuracy as well as placing a fraud alert on your file with one of the national credit reporting agencies.
